HomeMy WebLinkAbout2471-SLGENERAL NOTES FOR WORK 1:N THE CITY OF ONTARIO-(!S-1 THROUGH TS 1 ALL WORK SHALL CONFORM TO CALTRANS STANDARD PLANS AND STANDARD SPECIFICATIONS, LATEST EDITIONS, 2014 CALIFORNIA MANUAL ON UNIFORM TRAFFIC CONTROL DEVICES (CAMUTCD), CITY OF ONTARIO SPECIAL PROVISIONS AND STANDARD DRAWINGS, AND APWA STANDARD SPECIFICATIONS FOR TRAFFIC SIGNALS PR'EPARED: n SPECIFICALLY FOR THIS WORK. 2. ANY CONTRACTOR PERFORMING WORK ON THIIS PROJECT SHALL FAMILIARIZE THEMSELVES WITH THE SITE AND SHALL BE 1 SOLELY RESPONSIBLE FOR ANY DAMAGE TO EXISTING FACILITIES RESULTING DIRECTLY OR INDIRECTLY FROM THEIR OPERATIONS, WHETHER OR NOT THE FACILITY IS SHOWN ON THESE PLANS. 1 ALL OBSTRUCTION'S WITHIN THE AREA TO BE IMPROVED SHALL BE REMOVED ANb/OR RELOCATED AT THE DIRECTION OF THE CITY OF ONTARIO ENGINEER, UTILITIES ARE TO BE RELOCATED BY THEIR RESPECTIVE OWNERS UNLESS NOTED OTHERWISE. UTILITIES LINE LOCATIONS WERE TAKEN FROM AVAILABLE RECORD DATA AND WERE NOT LOCATED IN THE FIELD, UNLESS OTHERWISE NOTED ON THE PLAN. THE CONTRACTOR IS REFERRED TO SECTION 5 OF THE STANDARD SPECIFICATIONS. 5. IN CASE OF ANY ACCIDENTS INVOLVING SAFETY MATTERS COVERED BY SECTION 64-09(b) IMMEDIATELY OF THE CALIFORNIA LABOR CODE, THE CONTRACTOR SHALL I MI DIATELY NOTIFY THE STATE DIVISION OF INDUSTRIAL SAFETY. 6. STATE LAW (S,B 3019) REQUIRES THE CONTRACTOR TO CONTACT UNDERGROUND SERVICE ALERT AND OBTAIN- AN IDENTIFICATION NUMBER PRIOR TO THE ISSUANCE OF CITY'S ENCROACHMENT PERMIT. THE CONTRACTOR SHALL NOTIFY UNDERGROUND SERVICE ALERT AT 1- CEO- A MINIMUM OF 48 HOURS IN ADVANCE OF ANY CONSTRUCTION ACTIVITIES. 7. PRIOR TO THE START OF ANY WORK THE CONTRACTOR SHALL OBTAIN A BUSINESS LICENSE FROM THE CITY OF ONTARIO. THE CONTRACTOR SHALL ALSO OBTAIN AN ENCROACHMENT PERMIT FROM THE ENGINEERING DEPARTMENT NO LESS THAN 48 HOURS PRIOR TO START OF ANY CONSTRUCTION WITHIN THE PUBLIC RIGHT-OF-WAY, 8. THE CONTRACTOR SHALL OBTAIN A TRAFFIC CONTROL PERMIT 48 HOURS PRIOR TO TIME E OF CONSTRUCTION WITHIN THE PUBLIC RIGHT-OF-WAY. THE CONTRACTOR IS REFERRED TO SECTION 7-10.3 OF THE STANDARD SPECIFICATIONS FOR PUBLIC WORKS CONSTRUCTION. 9. A CITY ACCEPTED/APPROVED SET OF PLANS SHALL BE KEPT ON THE JOB SITE AT ALL TIMES, 10. A PRE -CONSTRUCTION MEETING SHALL OCCUR PRIOR TO CONSTRUCTION. ATTENDEES SHALL INCLUDE A CITY REPRESENTATIVE AND THE CONTRACTOR WHO WILL PERFORM THE WORK. "CUT -SHEETS" SHALL BE PROVIDED TO THE CITY AT THIS MEETING FOR ITS REVIEW, 11. THE CONTRACTOR IS RESPONSIBLE FOR VERIFICATION OF ALL EXISTING UNDERGROUND UTILITIES, WHETI,4ER OR NOT THEY ARE SHOWN ON THESE PLANS. THE CONTRACTOR SHALL CONTACT USA (800 - 642-2444-) AT LEAST 48 HOURS BEFORE BEGINNING WORK. THE CONTRACTOR SHALL POTHOLE ALL PROPOSED TRAFFIC SIGNAL LOCATIONS TO VERIFY/CONFIRM EXISTING UNDERGROUND UTILITIES, PRIOR TO ORDERING TRAFFIC SIGNAL EQUIPMENT. Y 12. THE CONTRACTOR SHALL GIVE 48 HOURS NOTICE FOR APPROVAL OF PROPOSED EQUIPMENT LOCATIONS. 14. ALL PULL BOXES ADJACENT TO MAST ARM POLES, OR WITH FOUR OR MORE CONDUITS ENTERING SHALL BE #6. ALL OTHERS SHALL BE #5 UNLESS OTHERWISE SHOWN, 15. THE CONTRACTOR SHALL CONTACT THE CITY OF RANCHO CUCAMONGA AND THE CITY OF ONTARIO ENGINEER'S OFFICE FOR APPROVAL 7 DAYS PRIOR TO ANTICIPATED ENERGIZING OF THE NEW SIGNAL EQUIPMENT. 16. CONTRACTOR SHALL SUBMIT TO THE CITIES OF RANCHO CUCAMONGA AND ONTARIO FOR REVIEW AND APPROVAL A TRAFFIC CONTROL PLAN FOR SIGNAL CONSTRUCTION (INCLUDE WORK HOURS, DATE, LANE CLOSURE, AND PEDESTRIAN AND VEHICLE TRAFFIC CONTROL). ANY TRAFFIC SIGNAL SHUT DOWN AS REQUIRED FOR ALTERATION TO ELECTRICAL SYSTEM SHALL BE LIMITED TO WEEKENDS OR LATE NIGHT ON WEEKDAYS (11:00 PM TO 5:00 AM). 17. ALL PEDESTRIAN SIGNALS SHALL BE THE COUNTDOWN LED TYPE. SHALL BE POLARA BULLDOG BDL3-G OR 18. PEDESTRIAN PUSH BUTTON ASSEMBLIES APPROVED EQUAL. SEE SPECIAL PROVISIONS, 19. FULL-SIZE LAYOUTS FOR EACH RSNS LEGEND AND ONE SAMPLE PANEL SHALL BE SUBMITTED TO THE CITY TRAFFIC ENGINEER FOR APPROVAL PRIOR TO FABRICATION. SEE SPECIAL PROVISIONS. 20. ALL WORK RELATED TO ELECTRIC SERVICE SHALL BE : PERFORMED IN CONFORMANCE WITH THE REQUIREMENTS OF THE SOUTHERN CALIFORNIA EDISON COMPANY. OBTAIN APPROVAL FOR EXACT EQUIPMENT, POLE, AND DETECTOR LOCATIONS PRIOR TO FINAL PLACEMENT. 21. DETECTOR LOOPS' SHALL BE SPLICED IN SERIES, PHYSICALLY ADJACENT DETECTOR LOOPS SHALL BE WOUND WITH OPPOSITE ROTATION. ROTATION REVERSAL MAY BE ACCOMPLISHED AT THE PULL BOX BY REVERSING THE LEADS. < 22. AT THE DIRECTION OF THE ENGINEER, THE CONTRACTOR SHALL DELIVER AND UNLOAD ANY SALVAGE AT THE ONTARIO MUNICIPAL SERVICE CENTER, 1425 SOUTH BON : VIEW AVENUE, OR CITY WELL SITE NO. 9, ON THE WEST SIDE OF COLUMBIA AVENUE NORTH OF DEODAR STREET. 23. PRIOR TO THE START OF CONSTRUCTION THE CONTRACTOR SHALL OBTAIN FROM THE CITY OF ONTARIO A SET OF SPECIAL PROVISIONS PREPARED SPECIFICALLY FOR THIS 'WORK. GENERAL NOTES FOR WORK IN THE CITY OF RANCHO CUCAMONGITS - THROUGH TS-3). 1.
