FIRE PROTECTION DISTRICT CLOSED SESSION MINUTES
II A. CALL TO ORDER II
The Rancho Cucamonga Fire Protection District held a closed session on Wednesday, June 16, 2004, in
the Tapia Room of the Civic Center located at 10500 Civic Center Drive, Rancho Cucamonga, California.
The meeting was called to order at 5:30 p.m. by President William J. Alexander.
Present were Boardmembers: Rex Gutierrez, Robert J. Howdyshell, Donald J. Kurth, M.D., Diane
Williams and President William J. Alexander.
Aisc present were: Jack Lam, City Manager; Pamela Easter, Deputy City Manager; James Markman,
City Attorney; Larry Temple, Administrative Services Director; and George Rivera, Administrative
Services Manager.

Reqular Meetinq
A regular meeting of the Rancho Cucamonga Fire Protection District was held on Wednesday, June 16,
2004, in the Council Chambers of the Civic Center located at 10500 Civic Center Drive, Rancho
Cucamonga, California. President William J. Alexander called the meeting to order at 7:09 p.m.
Present were Boardmembers: Rex Gutierrez, Robert J. Howdyshell, Donald J. Kurth, M.D., Diane
Williams and President William J. Alexander.

RESOLUTION NO. FD 04-017
A RESOLUTION OF THE BOARD OF DIRECTORS OF THE RANCHO
CUCAMONGA FIRE PROTECTION DISTRICT, RANCHO
CUCAMONGA, CALIFORNIA, MAKING AN APPOINTMENT OF ITS
REPRESENTATIVE(S) TO THE PUBLIC AGENCY SELF-INSURANCE
SYSTEM BOARD OF DIRECTORS
D7. Approval to proceed with the biennial review of the Fire District's Conflict of interest Code.
MOTION: Moved by Howdyshell, seconded by Kurth to approve the recommendations in the staff reports
contained within the Consent Calendar. Motion carried unanimously 5-0.
