A RESOLUTION OF THE BOARD OF DIRECTORS OF THE
RANCHO CUCAMONGA FIRE PROTECTION DISTRICT,
RANCHO CUCAMONGA, CALIFORNIA, MAKING AN
APPOINTMENT OF ITS REPRESENTATIVE(S) TO THE PUBLIC
AGENCY SELF-INSURANCE SYSTEM BOARD OF DIRECTORS
WHEREAS, the Rancho Cucamonga Fire Protection District is a member agency
of a Joint Powers Authority for purposes of worker compensation self-insurance and
administration entitled the "Public Agency Self-Insurance System" (PASIS); and,
WHEREAS, the District is required from time to time to update its primary and
alternate representatives to the governing Board of Directors of PASIS.
NOW, THEREFORE, THE BOARD OF DIRECTORS OF THE RANCHO
CUCAMONGA FIRE PROTECTION DISTRICT does hereby resolve to appoint their primary
and alternate representatives to the governing Board of the Public Agency Self-Insurance
System as follows:
Primary: Risk Management Coordinator
Alternate: Fire Chief
Alternate: Senior Accountant
