GENERAL NOTES AT GRAPELAND ELEM. SCHOOL AND ETIWANDA INTERMEDIATE SHEET - 2 SIDEWALK INSTALLATION PLAN AT 7110 AND 7150 ETIWANDA STREET
1. All work shall conform to the City of Rancho Cucamonga Standard Drqwings and the Standard Specifications for Public
Works Construction, (Green Book), 2012 Edition with 2013 supplements
2. Public and Private utilities and substructures shown hereon are from available records. The Contractor shall be
responsible for protecting all utilities and substructures as shown within the construction limits of this project and shall
be responsible for any damage as a result of the construction operations.
3, The Contractor shall notify Underground Service Alert 1- 800 - 422 -4133 and the following utilities two working days prior
to construction or regarding any problems.

Note: Notify Steve Bradley of WSOG 0 (909)392 -6137, at least two working
days prior to starting any work in the Vicinity of MWD facilities.
4. The contractor shall notify the City of Rancho Cucamonga Police Department, The Clty of Rancho Cucamonga Fire
Department and any affected school district, 72 hours prior to construction indicating any detour and /or road closures.
5. For inspection of this project, the Contractor shall contact the Public Works Section at (909) 477 -2740, Monday —
Thursday, and at least two working days prior to the construction start date.
6. A permit shall be obtained from the City of Rancho Cucamonga prior to any construction or encroachment within any
City of Rancho Cucamonga easement or rights —of —way.
7. During the construction operations at least one 12' lane in each direction shall be provided for each direction of travel
on all streets at all times. The traffic lanes shall be maintained on pavement and shall remain unobstructed.
8. The Contractor shall familiarize himself with all underground utilities, pipes and structures, and shall take sole
responsibility for the repair /replacement, at no cost to the City, for any damage to said underground utilities, pipes and
structures.
9. Contractor shall verify all conditions and dimensions shown on the plans at the site prior to commencement of any work.
All discrepancies shall be reported for direction immediately to the City.
10. On the date and at the time of the submittal of the Bidder's Proposal the Prime Contractor shall possess any and all
contractor licenses, in form and class as required by any and all applicable laws with respect to any and all of the work
to be performed under this contract; Including but not limited to a Class "A" License (General Engineering Contractor) or a
Specialty Class "C -12" license (Earthwork and Paving Contractor) sufficient to cover all the work to be performed by the
Prime Contractor in accordance with the provisions of the Contractor's License Law (California Business and Professions
Code, Section 7000 et. seq.) and rules and regulations adopted pursuant thereto.
EXISTING SURVEY MONUMENTATION
All existing survey monuments that will be disturbed by this project shall be referenced and
reset by the City Construction Surveyor in accordance with Section 8771 of the California
Surveyors Act. Found monuments are shown on the plans and described as follows:

STRIPING GENERA_ L_ NOTES (N /A)
1. All details and dimensions for traffic striping and markings and the installation thereof shall conform
to Caltrans, Standard Specifications, Traffic Manual, Maintenance Manual and Standard Plans and City
Standard plan no. 133, unless otherwise approved or directed by the City Engineer.
2. Procedures for painting of striping and pavement markings and installation of pavements markers shall
conform to the Standard Specifications for Public Works Construction and the following provisions.
A. All pavement markings shall conform to the city maintenance stencils, (S,A,E) unless otherwise noted.
B. All conflicting traffic striping and pavement markings shall be removed by WET SANDBLASTING.
C. Sufficient control points and spotting removed during sandblasting shall be replaced prior to installation
of striping and markings.
D. All control points and spotting removed during sandblasting shall be replaced prior to installation of
striping and markings.
E. All control points and spotting shall be inspected and approved by the City Traffic Engineer prior to
installation of new or replacement striping and markings.
F. All striping and pavement markings installed on new road surfaces shall be approved low VOC type,
15 mil thick (wet). Installation shall be in two coats with the second a minimum of two weeks after the
first, unless otherwise noted.
g. All thermoplastic striping and pavement markings shall be installed in one coat and be 10 mil thick (dry),
unless otherwise noted.
H, Under no conditions shall a roadway be left over night, weekends or holidays without appropriate striping
and markings to the satisfaction of the City Engineer.
I. All traffic striping and pavement markings shall be installed with all appropriate permits and traffic control
measures, in accordance with the city and state standards.
J. Temporary or construction striping and markings shall be in accordance with the city and state standards
and as directed by the City Engineer.
3. Street Name Signs shall be installed per City Standard Plan No. 401 -A.
4. Traffic Signs shall be installed per City Standard Plan No. 401 -B.

1. General Conditions:
a) An RRFB shall consist of two rapidly and alternately flashed rectangular yellow indications having LED -array based pulsing light sources,
and shall be designed, located, and operated in accordance with the detailed requirements specified below.

b) The use of RRFBs is optional. However, if an agency opts to use an RRFB under this Interim Approval, the following design and
operational requirements shall apply, and shall take precedence over any conflicting provisions of the MUTCD for the approach on which

3. Sign /Beacon Assembly Locations:
a) For any approach on which RRFBs are used, two W11 -2 or S1 -1 crossing warning signs (each with RRFB and W16 -7p plaque) shall be
installed at the crosswalk, one on the right -hand side of the roadway and one on the left -hand side of the roadway. On a divided
highway, the left -hand side assembly should be installed on the median, if practical, rather than on the far left side of the highway.

b) An RRFB shall not be installed independent of the crossing signs for the approach the RRFB faces. The RRFB shall be installed on the

4. Beacon Dimensions and Placement in Sign Assembly.

5. Beacon Flashing Requirements:
a) When activated, the two yellow indications in each RRFB shall flash in a rapidly alternating "wig -wag" flashing sequence (left light on, then
right light on).
b) As a specific exception to 2003 MUTCD Section 4K.01 requirements for the flash rate of beacons, RRFBs shall use a much faster flashes

rate. Each of the two yellow indications of an RRFB shall have 70 to 80 periods of flashing per minute and shall have alternating but
approximately equal periods of rapid pulsing light emissions and dark operation. During each of its 70 to 80 flashing periods per minute,
one of the yellow indications shall emit two rapid pulses of light and the other yellow indication shall emit three rapid pulses of light.
c) The flash rate of each individual yellow indication, as applied over the full on -off sequence of a flashing period of the indication, shall not
be between 5 and 30 flashes per second, to avoid frequencies that might cause seizures.
d) The light intensity of the yellow indications shall meet the minimum specifications of Society of Automotive Engineers (SAE) standard J595
(Directional Flashing Optical Warning Devices for Authorized Emergency, Maintenance, and Service Vehicles) dated January 2005.
6. Beacon Operation:
a) The RRFB shall be normally dark, shall initiate operation only upon pedestrian actuation, and shall cease operation at a predetermined
time after the pedestrian actuation or, with passive detection, after the pedestrian clears the crosswalk.

b) All RRFBs associated with a given crosswalk (including those with an advance crossing sign, if used) shall, when activated, simultaneously
