CAIiFORNIA, MAKING AN APPROTIMMqT OF ITS
REPRESENTAT/VE(S) TO THE PUBLIC AGMqCY S~.F-
INSURANCE SYST[M BOARD OF DERS
WHEREAS, the Rancho Cucamonga Fire Protection District is a member
agency of a Joint Powers Authority for purposes of worker conloensation self-
insurance and administration entitled the "Public Agency Self-Insurance
system" (PASIS); and
WHEREAS, the District is required from time to time to update its
primary and alternate representtives to the governing Board of Directors of
PASIS.
NOW, THEREFORE, the Board of Directors of the Rancho Cucamonga Fire
Protection District does hereby resolve to appoint their primary and alternate
representatives to the governing Board of the Public Agency Self-Insurance
system as follows:
L. Dennis Michael, Fire Chief
Bob Dominguez, Administrative Services Director
PASSED, APPROVED, and ADOPTED this 18th day of November, 1992.

District do hereby certify that the foregoing Resolution was duly passed,
aiDproved, and adopted by the Board of Directors of the Rancho Cucamonga Fire
Protection District, at a regular meeting of said Board held on the 18th day
