A RESOLUTION OF THE BOARD OF DIRECTORS OF THE
RANCHO CUCAMONGA FIRE PROTECTION DISTRICT,
RANCHO CUCAMONGA, CALIFORNIA, MAKING AN
APPOINTMENT OF ITS REPRESENTATIVE(S) TO THE PUBLIC
AGENCY SELF-INSURANCE SYSTEM BOARD OF DIRECTORS
WHEREAS, the Rancho Cucamonga Fire Protection District is a member agency of a
Joint Powers Authority for purposes of worker compensation self-insurance and administration
entitled the "Public Agency Self-Insurance System" (PASIS); and,
WHEREAS, the District is required from time to time to update its primary and alternate
representatiyes,t6the governing Board of Directors of PASTS.
NOW, THEREFORE, the Board of Directors of the Rancho Cucamonga Fire Protection
District does hereby resolve to appoint their primary and alternate representatives to the
governing Board of the Public Agency Self-Insurance System as follows:
Primary: L. Dennis Michael, Fire Chief
Alternate: Joe Sanchez, Risk Management Coordinator
• PASSED, APPROVED and ADOPTED this 17th day of January, 2001.

District, do hereby certify that the foregoing Resolution was duly passed, approved and adopted
at a regular meeting of the Board of Directors of the Rancho Cucamonga Fire Protection District
